I have 1 awesome form template that I would like to use on a lot of pages without having to clone the form. I can include the embed code in my PHP page template, but I won't be able to tell on which specific page the form was submitted. Is there a way to use my site PHP to add the specific page title or URL to the form on submit?

To confirm, you would like to use the same form on different websites but you would like to know what site your submissions are coming from?
If that is the case, this can be achieved when using the iFrame embed method and a hidden field on your form.

That is exactly what I needed. The Power Tools section was replaced with the Widget section, so that's the only thing that deviated from the link you provided. Thank you so much!
