At the end of this form (via “Submit”) there is supposed to be an e-mail that goes to the parties with the data entered. It was working, now it is not. The responsible party at the State Agency was notified so maybe it is now corrected or maybe they will work on it Monday. Not sure. But if you see a glitch, maybe you can suggest the fix. Internal e-mail server submit glitch?
Might have just misguided you. The link below is to start the second step of form completion, This submit button is working.. It is the second submit button at the end of about 6 pages of input that is not working.
Thanks. Is there a way to have a dialog and do some testing? I checked with GoDaddy and they run a blacklist so the suspicion is that a jotform smtp server has been blacklisted. I tried a different e-mail address and server and curiously still had problems.
This might be the form giving me trouble.
