How to modify format of submission to look like an agreement?

Is it possible to reformat and modify a submission into another format (such as an agreement)?

I am not sure if I have understood your question correctly. Do you mean to say that you want your form's submission to look like an agreement?

Well it is not possible to change the format of form's submission but it is possible to update your form's submission email template to look like an agreement. You can then simply print this email in pdf or save it in your inbox. Please check the screenshot below on how to edit your email template:

Another option is to use "Narrative Fields" widget instead. This will allow you to create a form like a fill in the blanks form.

Thanks - I was hoping there was a way to take the information the user submitted and represent it in a new layout (such as an agreement)
