I have just opened the form to continue editing, but it appears as if the form is completely empty - as if I was starting a new form.
I have checked revisions and all shows okay. Also, the form output is okay:

Please note - this is very urgent. I am not sure whether any Support personnel have viewed this problem yet.
If someone has looked and even if they do not know how to fix the problem, could they at least please acknowledge the problem?
It is not as if there is a workaround - I am completely at a loss as to why the form is empty to edit.

Hi,
Apologies for the delay. You can fix this by removing the custom CSS code.
1. Open your form in Form Builder and navigate to the Form Designer, even though you're seeing blank. Click the CSS tab and remove the custom CSS code there.
2. After that, it will display your form properly both in Designer and the form builder.
If you need the custom CSS code, I would suggesting you check the errors and make a fix on it.

I have removed it.
That was a simple fix, but I am concerned because I never manually added the following CSS code:
label[for="input_315_0"],label[for="input_319_0"],label[for="input_320_0"], ,label[for="input_327_0"]{visibility : hidden !important;}
Is there some option I must have accidentally selected that might have injected this code because I don't even understand this code, let alone having manually added it? I'm concerned if this has been added by someone else unless someone has somehow guessed my login and interfered with the form.
Any suggestions as to how this code might have appeared?

Hi,
I believe it is not manually added by someone. I've seen this code injected with some of our forms but I haven't been able to reproduce it manually yet, it seems like a series of editing using the settings in the form builder and the Form Designer or probably a hiccup during an update. I haven't been able to reproduce it in my end but will try to investigate further. Thank you for your concern, if you are still worried, you can change your password here Account->Profile->Password.
