I have just added a file upload field to my form. The uploaded file appears in submissions on this site but is not forwarded to me via email along with the rest of the submission information.

Your email configuration is correct and our email logs show successful form email deliveries. You can check your email history by following this guide: http://www.jotform.com/help/293-How-to-view-all-your-form-Email-History.
I'm sure the form emails have landed to your spam folder so kindly check that. If the emails are into your spam folder, mark them as not spam and add email@example.com to your contact or safe sender list. Check the emails using your webmail access instead of your email client software.
