Form doesn't show on iPhone or iPad.

my problem is that my form doesn't seem iPhone and iPad. İt's only working on PC or Mac.
please help me. 

I tested and could replicate the same situation. 

Please try embedding the form using the iFrame method to see if it solves.

Thank you Carina, it is seem on my iPhone and iPad but another problem arose. :(

From working on Firefox but it doesn't work properly on safari, iPhone and iPad . 
When you want to write a message on Form it is sending you to top of the page.

if you test it on your browser you will see the problem what I mean.

Yes I could replicate the same situation. I made some research and unfortunately there seems to be reported issues regarding Safari and iFrames, so probably this is the cause. iFrame is a recommended embedding method as it solves most script conflicts between the form and the site, but it doesn't seem the case here.
