I see what you mean. You can't remove the integration because the "Remove Integration" button disappears after a few seconds when it automatically logs in your existing account.
I will need to report this bug to our developers. We will receive a notification from them when this has been fixed.
For an alternative, you can remove the integration by cloning your form. Please check this guide on how to do that: How To Clone an Existing Form From Your Account. The cloned form should have remove all integration, from there you can use that one instead.
Let us know if this works.
