Form unavailable error upon submission

    Hi - several of my forms are coming up as 'form unavailable' when someone clicks submit. See photo:

    Also, in my list of forms, some are faded. These ones do not work.

    When you see a grayed out form, it means that the form is currently disabled. Please note that you can Enable/Disable the forms at any time.
