I do apologize, I do not completely understand your request. Is it that you would like the option to turn an image to a link within your form?
If that is the case, this can be achieved using the "Text" field.

If this is what you are looking for. You would have to source the image that you would like.
You would then have to add a text field to your form.
You would then select to edit the text and select the "HTML" to add the desired code
