The card you had on file had expired. I am trying to upgrade it and do not know how.
Also, how can I verify what type of account I initially signed up for? Was it premium or which one was it?

Upon checking your account, I found that your premium monthly subscription is already cancelled.
I would suggest you to click on the following URL to upgrade your account again using your new credit card: http://www.jotform.com/upgrade?username=NVESQ&account_type=PREMIUM&period=monthly
