FTP form submission not listing folders and also cant upload

I am trying to set-up ftp submissions upload but it is not listing directories nor uploading files to any of folder manually entered.

I checked our FTP connection wizard to make sure it was still working and it appears as though it is.  I did need to check the box for "Is this FTP passive" in order for the files and such to show up:

Without that box checked, I did not have anything show up in the next screen.  With it checked, I was able to see the files in my FTP site:
