Form has converted back to its previous format/design

The Jotform I created for Spring 2015 wellness program registrations has converted back to the original form I cloned when creating it.  Last week the form was correct, and I have over 50 confirmation emails from participants who registered correctly. However, now that the form has converted back I only have a list of names of individuals who have registered so far, but I missing all the data on what they signed up for because it lists Fall 2014 options. 

How can I recover this form and all my submissions?

Since when this issue happen to the form? Do you remember some of the fields that went missing, if there's any?
