Our web developer used JotForm to send online information requests from our new website, which we receive in an email.  The JotForm Salesforce integration appears to be able to map JotForm fields with SF fields to create a new lead in SF.  Is it limited to name address, phone as in the example, or will it match up to all the fields created in my customized JotForms, or can I add and map the custom fields?

    Sample JotForm format:

    Question Answer
    Business Name MAXBURST
    E-mail test@test.com
    Phone Number (630) xxxx100
    Cell Phone (630) xxxx100
    Start Up? Yes
    Time In Business 3Months
    Revenues-Average Monthly 300,000
    Credit Level Estimate B 650-699
    Nature of Business test
    Equipment to Lease/Finance test
    Make test
    Model test
    Equipment New
    Cost $
    Purchased Through Vendor
    Planned Down Payment

Yes, it is possible to customize the Salesforce integration, but for this to work it is necessary to have the same field types in Jotform as in Salesforce.

You can see here more details on Salesforce field types. You can also see this guide on how to create custom fields.
