How do I get this form to open on my website?

    I have a few problems with the form on my website that I need an answer for. The first is how do I get this form to open on my site when the Become A Reseller button is pressed without having to click the box after the Resell Button?

    When the form does open after clicking on the Reseller Form Box, it does open and one can complete the form:however,when the form comes to my email address, part of it is missing. It is a 2 or 3 part form and the second half does not follow the first page.

    Hi Harold,

    I can't seem to find the "Become A Reseller" button to the top of the Youtube video. Did you remove that?

    You can open the form in a lightbox and use the button to trigger it:
