    how can i get the value of the user in input tag?

    Like in first name, middle name, last name, phone number. when i try to get their value the result was null.

    Glenn,

    Is this something to do with pre-population? If so, the values of the name attribute should not include the first four characters. Also, the format should be in the form of {field:sub-field}. For instance, in the code you provided above, the value of the name attribute for the first name field is q15_name15[first]. So, to get the value entered their you would remove the q15_ and then change name15[first] to {name15:first}.
