Large Button Answer Options (Single question with YES/NO answer option)

    Looking to create a single form, where the user can select Yes or No to one question. This will be used on a touchscreen, so ideally the Yes/No answer buttons would be large like submit buttons.

    Also, this same form will be used in 5 locations that should each be automatically identified in reports (hidden field value to identify form)?

    You might consider the following widget fields to have the button-styled radio buttons:

    - Button Radios

    - Bootstrap Switch Field

    - Line Radio Buttons
