How do I remove a 'sample logo' image at the top of a form?

I created a form and a random 'sample logo' image is visible at the top and I cannot remove it. I cannot find this in the source code either. I would like this removed.

You can remove the cover logo or image that you added by following the screenshot below.

 -Open the form on Form Designer->Click the Cover image-

-Navigate to Form Cover section->Click the Remove button
