Form that can repeat the set of questions a number of times

We have previously used IAuditor, It gives us the ability to have a set of questions like below and then to 'add equipment' to then repeat the set of questions a number of times that is required.

We are looking at other programs that could give the same functionality.  Our aim is to give our clients a overview of their equipment at their property and instead of people submitting a number of forms if we are able to have all the information on one form that would work well.

It is based around safety reports but is customizable.

So i have done a rough draft of type of questions we are looking for in word, not sure how to upload this so i have taken a screen shot.

The reason i need these form to be dynamic and really customisable is we are not sure how many equipment we would be adding for one site.

File Upload is currently not possible in the configurable list widget. I think I would suggest going for a workaround such as adding a separate file upload field to the form.

For example, our default file upload field. You can allow multiple file upload to it so users can just upload all the photos at once.
