Just to update everyone this issue is still going on. Cannot upload docs using the upload feature on android device. 1-15-2015

I didn't include enough information. Sorry.
Android Kitkat running chrome browser.
Upon clicking upload and fetching file, you get a "Bad File Extension" error.
The only case in which it will work is if you browse through your documents through the "File Manager" on the android device.
Tested on the Sony Experia Z1s and Droid Turbo - Both running Kitkat and the default chrome browser.

Can you please confirm that this is the jotform http://www.jotformpro.com/form/50094786247968 you are having issue?
I checked the submission of the form, and I noted that it has only one test submission but did not used the upload field. The upload field on the form is not required, so it can be submitted even if you do not use the upload.
I made a test submission and it went through and can see it on the submission data of your form.
Can you please try submission again. Let us know if issue persist.

Thank you for providing more details.
Can you also try changing the 'File Extensions' allowed property of the Upload field to asterisk(*) , this is to allow any file type/format on the upload.

Here is the error I get when trying to upload the file from ANDROID DEVICES.
Chrome Browser. This was after adding the Asterisk * to the allowed file extensions

I was able to replicate the same issue on the "no extension" error. It seems it doesn't work with asterisk(*) extension anymore.
BUT I was able to make it work on my Android device also when I remove and add back the upload field with everything on default only.
can you please try one last time,. remove and add back the Upload field on your jotform and I will test also again.
