Is there a way to add sub check boxes?

    We are creating a form which would gather the payment options accepted by our clients, who are primarily dentists. We would like to list all the insurance providers in the main check boxes and if they click one or more of the main check boxes, the different plans offered by each insurance provider would then show as sub check boxes. Any suggestions on how we can achieve this? Your kind assistance on this matter is greatly appreciated.

    Hello Chris,

    I am not sure if I have understood your question correctly. Do you mean to say that you want to display a set of check box when another check box is selected?

    Yes it is possible to achieve your requirement by taking advantage of our "Conditions" feature. You need to actually add conditions in your form to show/hide form fields based on the click of another form field.
