I get problem with the languages of the form "LIDO - Petra"
I have verifed with Safari, Firefox, Chrome & Opera.
When I open the soft it do not work. I should close the window (no quit) and then I open again, it works.
Do you know a way to fixe it ?

I tried checking your form and the language option is showing properly. I checked the form using Internet Explorer 11, Mozilla Firefox and Google Chrome.
Here's what you can do:
1. Log out from JotForm.
2. Please clear your browser cache.
3. Close and reopen the browser.
4. Log back in to JotForm.
