Yes or No Questions not working with Conditions

    I have a very simple scholarship template with 5 yes or no questions.

    I cannot figure out how to make the conditions work for me & I'm web designer!

    Basically, I want there to be two Thank you responses - one that tells the person they qualify for a scholarship, and the other thank you response telling them they don't.

     

    Here are my 5 questions:

    Does your child qualify for an IEP, MET, or 504 Plan? [yes or no]

    Was your child adopted or in Foster Care? [yes or no]

    Does your child attend a D/F public school? [yes or no]

    Are you currently an active member in the military [ yes or no]

    Are you currently a resident of Arizona? [yes or no]

     

    The person must say yes to living in Arizona & yes to one of the other 4 questions to qualify. If they yes to multiple options, great too. If they say yes to living in AZ but no to all other questions, they don't qualify.

    How do I set this up? HELP PLEASE!

    Hi,

    Thank you for contacting us for support. I am more than happy to help out. Base on my understanding of your situation, you would like a series of Yes/No questions to determine if an applicant is qualified for your scholarship program. I have analyzed and test your form with your current conditions and I was able to replicate the issue you are having.

    However, I was able to make some adjustments to both the form and your conditions to make it fully functional in my cloned form. I appointed a score to each question. Once all the Yes/No questions are selected, all the points are being summed and a condition is being used to determine the thank you page the form user will see after submission.

    You can clone my form and make further changes if you wish. Multiple conditional statements were set to cover all the possible combinations. I have concluded that once you are not a resident of AZ you are automatically disqualified for the Scholarship base on the conditions you provided initially.

    My form was configured base on the conditions below.

    To Quality:
    Does your child qualify for an IEP, MET, or 504 Plan? Yes
    Was your child adopted or in Foster Care? Yes
    Does your child attend a D/F public school? Yes
    Are you currently an active member in the military Yes
    Are you currently a resident of Arizona? Yes

    If the applicant is a resident of Arizona and selects yes to one or all the other questions above, they would qualify for the scholarship.

    To NOT Quality:
    Does your child qualify for an IEP, MET, or 504 Plan? No
    Was your child adopted or in Foster Care? No
    Does your child attend a D/F public school? No
    Are you currently an active member in the military No
    Are you currently a resident of Arizona? Yes

    If the applicant is a resident of Arizona and selects no to all the other questions above, they would NOT qualify for the scholarship.

    --OR--

    Does your child qualify for an IEP, MET, or 504 Plan? Yes
    Was your child adopted or in Foster Care? Yes
    Does your child attend a D/F public school? Yes
    Are you currently an active member in the military Yes
    Are you currently a resident of Arizona? No

    If the applicant is NOT a resident of Arizona and selects YES to ALL or NO to ALL the other questions above, they would NOT qualify for the scholarship.

    If the above condition is not valid, it can be modified after you have cloned my form. If assistance is need we will definitely given a helping hand.

    When all the selections are made, the sum of the values in each of the appointed text boxes are being calculated and stored in a number field (Total Score). The Total Score field along with the AZ residence question is being used in multiple conditional statements to determine which Thank you page the user is being directed to.

    NB: All the text boxes added to the form by me were hidden so the form user will have no knowledge of their existence.
