I have checked your account and form and noted that it was already reinstated.

I suggest also that you delete the form you had created http://www.myjotform.com/form/50155742740552 since it contains nothing and is being confused by the phishing filter as a phishing form. This was the reason your account was previously auto suspended by the phishing filter.
