See my option list the way it looks in jotform
now see how it looks in wordpress (Using Source)
But if I use embed or iframe , it looks great, but it creates a big long page because it has all forms, look
Defenitely, I NEED TO USE SOURCE because that one will not create a very long page, the only issue is the styling with the option buttons are out of wack.

I'm sorry for the trouble caused to you.
Have you tried our wordpress plugin already? Please try to embed your form using our wordpress plugin and see if that helps you: http://apps.jotform.com/app/form_generator_for_wordpress
If the issue persists, I would suggest you to embed your form using its iFrame embed code and then share the web page URL. We will give you the updated iFrame code with fixed height which should solve your problem.
We will wait for your response.

There seems to be some code conflict between your form script and other scripts available in your webpage. That is the reason why he condition you have added in your form to show hide the other forms in iFrame is not working.
Please use the following iFrame embed code and that should solve your problem:
<iframe id="JotFormIFrame" allowtransparency="true" src="//form.jotform.us/form/50157929370156" frameborder="0" style="width:100%; height:850px !important; max-height:850px !important;border:none;" scrolling="yes"></iframe>
Hope this helps.
