I recently created from in jotforms in my login details email@example.com I think around December 2014 but when I log in today I cannot found the form in my jotform account (I need to edit it) but the form is still on my website test page (http://www.apollokitchens.com.au/test-form.html) please let me know what to do.
Upon checking this form http://www.jotform.me/form/43473635299466 embedded to your website. It seems that you have created that form under a guest account. What I did is I moved that form including the data on it to your account with a username dkelso.
Please do log out and log back in to see the changes in your account.
