Calendar field is not being displayed everytime.

    We both also discovered the Calendar was at times not working or displaying either?

    We have tested on both Mac and PC in all web browsers with the same concerns.

    I'm sorry for the trouble caused to you.

    I did check your web page and I am unable to replicate the issue you are having. Your form does display the calendar field correctly without any problem. I did check your form in multiple browser and it seems to work as expected.

    Are you having this issue with any specific browser and OS? Do let us know and we will take a look. I would also suggest you to embed your form using its iFrame embed code and see if that solves your problem.
