I set up a form this morning and put it on my dev version of my site. I closed my browser, re-launched it a few minutes ago, logged back in to JotForm and my for is gone? Did I cause this? Can it be restored so I don't have to rebuild it again?

The form was created under a guest account generally these forms are not kept for long, once the browser cache and cookies are deleted forms would be removed as well. For now what you can do is to clone the form into your registered account so that you can manage the submissions and have access to the form.
