I checked your account and it appears to have been automatically suspended by our system. This auto-suspension happens when our filter detects fields in forms that can be fraudulent or phishing in nature such as forms with fields asking for financial information like credit cards, fields that ask for user access information such as a login form, or imitates a known website.
Upon reviewing your form I did not find anything that can malicious. So I'm suspecting that the word 'credit card' caused your account to be suspended. However I have now whitelisted your forms and have reactivated your account. Your forms should be accessible by now.
