Email Submissoin does not show correct questons and answers of the form

Hi i  used template for my form but when i receive notification of submission i don't see all my questions and answers but i can see old questions from the template i dont need. How can i receive  form with all my questions and answers? thanks

Please refresh the Notification Email of the form. This way all the fields from the form will be updated on the Notifier.
