It appears that email alerts are not being sent from my forms. Further, informaiton from our online application form seems to be coming/bouncing from an non-Jotform.com address. Can you advise?

First off, we apologize if it has caused uneasiness on your part. Indeed, we are using different domains to send form email alerts. This is because some mail servers have blocked our old mail server domains due to abusive behavior conducted by some of our users.
We are using the other domains so that the email alerts will not fail in getting delivered. You can be assured though that your form data are still very safe. These domains/addresses are still under our control and should not cause any security issue.

OK. Then the problem is these other domains are having a problem delivering the email to us. Could you please let me know what I need to unblock? or white list? for these new domains?

Kindly have these IP addresses whitelisted on your server.

I am also having this problem. Luckily, I discovered the problem after entering TEST DATA in a new form I created today. I'm receiving the Notification emails immediately, but I'm not receiving the Email Alerts that should be sent the 2 gmail addresses I use for testing purposes, both of which are MY gmail accounts and I receive/send from my main gmail Inbox.
I don't think I'm able to implement the above solution that Neil Vicente provided to Bill Hargrave. Perhaps Jotform needs to communicate to gmail to get this fixed.

EurekaBizSolutions: First of all please check the spam folder under gmail webmail service. If you do not see your mail alerts there, please provide us with the form ID in question. Then, we will be able to proceed with the further investigation.

No, the email alerts are not going into my gmail webmail spam folder. (I use gmail's webmail inbox to receive/send all my email.
The form ID# is 12830542033.

First off, I hope I am correct in assuming that the email address configured for your form's notification setup is not a Gmail address, and that your problem is about autoresponders not going through to the form submitter's Gmail address.
I inspected your autoresponder setup and found out that your recipient address is set to "email@example.com" when it should have been the e-mail field in your form, since autoresponders are supposed to be sent to the people who submitted the form.
This is the reason why the autoresponders are not being delivered to the Gmail address specified in the form submission. They are instead sent to firstname.lastname@example.org. Please correct this email setting. Click Setup & Embed > Email Alerts > Autoresponder > Reply-To and Recipient Settings.
To find proof that our new mail servers are working perfectly fine in conjuction with Gmail, I have added my personal Gmail address to your form notification recipient, then made a test submit on your form. Below is a screenshot of the email I received:
