configurable list results are not showing

Hi,
Are you referring to the Configurable List widget not displaying in your browser or the results of it is not being showed in your email notification?
I've checked your form to Google Chrome, Internet Explorer and Firefox and the widget seems to show properly.
I also tested it in Mac OS Yosemite withe Safari 8 browser, and it shows correctly.
Have you tried clearing your browser's cache and the form's cache? You may also try re-adding the widget to see if that fixes that problem.
