Hi,
Could you please share with the the form URL you're working at? I can't seem to find the form having a upload field.
You can add an upload field under the "Form Tools" section. You'll see below when you click the upload field and check its properties, you'll see different options where you can allow multiple uploads, set max file size and limit what file types to be allowed.
We also have a couple of upload widgets that you can use aside from the default file upload field. Here's the page for that: http://widgets.jotform.com/category/file_upload.
The uploaded photo or file will then be uploaded to our data centers.
If you would like to check if the uploaded file link is attached to your email, you can check it by:
1. Navigate to "Emails"->Click the existing notification setup.
2. Inside the "Compose Email" window you can add the values of each form fields. Here's the name of the field for my upload is "Upload", I added it using the form fields at the right giving me a tag {upload} in the content.
