The form I need to design has the option at the beginning to choose either single or couple in section 1 then in section 2 it either has 1 section or 2 depending if you are single or a couple. How can I display this on a form?

How can I create a form that gives me the option at the beginning to chose either single or couple options. I have attached a screen shot of the form

    Hi there,

You can create a form like that by using "conditions" on your form. You can either use form collapse, or page break for skipping pages depending on the user's answers.

I created a demo form that using the "Skip to a page" conditions. Here is the url of demo form: http://form.jotformpro.com/form/50224064755956

Here's a guide on how to clone a form.

This is the condition that I've used.

Please try to clone the form and check how it goes. I used "page breaks" and "conditions" on this form. Like I said, using form collapse is also an alternative.

Let us know if you need any help. Thank you!
