You must create a List and configure it on Mailchimp to add the extra email fields.
To do this, go to your List Name (on the image: Multiple Merge Test)
Settings > "List Fields and *|Merge|* Tags"
Make sure the added Field Type is set to "Text"
Then, on Jotform side,choose a list
And make sure that you have added a standard "Textbox" field and NOT an "Email Field" for the extra fields.
This should make the trick. I hope this helps with what you are trying to achieve.
