How to have Radio buttons as well as Check boxes in the same Purchase Order field?

I need help with this, I cant choose radio buttons as well as checkboxes for the purchase order field.

I wanted to do something like Parent (radio button) option and Child (checkboxes) option, in other words 3 radio buttons and 5 checkboxes. So the radio button will be required but the checkboxes will not, it will be additional.

Any help would be appreciated.

You will not be able to do it directly, but what you can do is set the fields to pass the total sum to your payment processing tool instead, which would allow you to use any field or widget to make your form look nicer while offering different options.
