how does report builder work? I can only see the results of two questions when i select build new report.

I am not sure if I have understood your question correctly. I did check your visual report builder and it seems to display the questions correctly. 

Please be noted that visual report can only show the chart for following field types only:

Drop Down

Radio Button

Check Box

Star Rating

Scale

Grading

Slider

Matrix 

Please be noted that by default only two charts charts will be displayed in the report. If you want to display more charts, you will have to drag & drop more charts in report. Please check the screenshot below:
