Thank you Welvin for reactivate my account.

Actually you have already seen that it looks like an account. I shall try to make another from consists one field named STUDENT ID (Provided by me) and using condition logic I will set it on that way, when this student enter his student ID and click in submit button then it will directly go to this account type form link, where I can upload notes for the particular student.

I hope your system will allow me to do so. 

I see. Well, you can create that kind of form. For as long as your form doesn't violate our Terms of Use, then you're safe from suspension.
