Hi,
I checked your notifications and it appears as though some changes may have been made to the fields within. When I re-created the notification it did indeed come through with all the fields:
Since I am unsure exactly which part of your current notification is causing the issue, it is probably best just to add a new one:

Adding a new one should link the proper field names with their data.
If this does not allow you to receive all your data in your notifications, let us know and we will be happy to have another look.
