I see that you are having some problems using the form designer and then embedding it but this mainly appears to be three separate issues so we'll deal with them one by one so that there isn't any confusion while we are helping you.

Problem #1 = In order to understand our Form Designer Feature I would highly recommend referring to our guide here http://www.jotform.com/help/300-Form-Designer-Tutorial-Let-s-create-fantastic-forms- to get a better understanding of how to use it properly so that you can get the results you desire.

As for your second and third problems those are technically different questions for each so I have split them into new topics.

Problem #2 = Go here http://www.jotform.com/answers/503642 for help with resolving the two different styles for your embedded jotform.

Problem #3 = Go here http://www.jotform.com/answers/503640 to learn how to revert your form.

I am having the exact same issue.  it is not you, it is Jotform!!

I have re-sent my issue to Jotform for review and provided endless documentation.

Let's see if we get a fix for the issue.  :-)
