How can I create a workflow form?

DISREGARD....Found solution in forum.

Does this form have the ability to be sent to other parties for approvals purposes prior to be submitted to the Source of record.

Use Case example:

Employee enters request >...Send to Supervisor >... Approve/Not approve > ... Submit to Head Manager (if approved ----  not approved route back to employee)
