Why does my email notification say " Appointment request from Medical website"

    Then the form says ' Eyecare Opticians Appointment Request

    Hi,

    I checked your forms' notification and the subject line is set to "Appointment request from Medical website":

    The line containing the header "Eyecare Opticians Appointment Request", is in the body of the actual email.

    You can modify these by clicking on the emails tab in the form builder, then clicking on your notification:
