I need the ability to have the customer enter a price which is different for each customer and I would like to sell additional items on top of the price they enter all from the same form.
You can use 'conditions' in order to do that. You can use the 'Form Collapse' or 'Page Break' under Survey tools. Please check this demo form that I created (http://form.jotformpro.com/form/50274966589977). Please take note that this is just a sample form about conditions in order to show you how it works.

I used the 'Show/Hide a form field' on the Conditions Wizard. This is the conditions that I created.
