    Account has been disabled

    I recently upgraded to a Premium account last Monday (the 26th) so that I could receive more submissions from Members of my organization.  However I received an email today saying that my account has been disabled and that I have reached the limits on my account.  I highly doubt that I received over 1000 submissions since Monday so I was writing to find out why my account had been disabled.  I need to have the form back up ASAP is it is on a deadline coming up.

    Hi Morgan,

    Unfortunately, you've made the payment without your username in the record so it says "undefined". But I've fixed this for you. Your account is now subscribed to our Premium Monthly Plan. Kindly login to the account to see the changes.
