It's been a long time since creating the form I have at Jotform, and when I tried to log in so that I can edit the form, the 'reset password' doesn't send any email instructions, just a form to sign up for a new account.
How can I recover my account access?

After reviewing your website, I am seeing that your form is currently on a guest account associated with the email address: email@example.com
This means that no account was created when the form was created. Please create an account using that email address to gain access to your form.
