We have several versions of the same form. We need to download a blank PDF of the form and all the other forms look fine except for this one. Since they are pretty much exactly the same, we are not sure why.

To my understanding I believe you are using the "Create a PDF Form" and this for some reason is making your first Organizational Leader Form small whereas your second Leader Form is fine.
I did some testing of this found that I was able to replicate the same issue as you. It seems to me that there is something wrong here with the form itself creating a pdf copy.
Have you tried doing this on a clone of your form to see if the same issue occurs?
Please allow me some time to try to investigate this more to see if I can find out what is happening with it.
Well I did do some poking around on this in a couple different tests but at the moment I'm unable to find a cause or even a reason why this might be happening.
When I was doing some tests of your jotform with the issue in comparison with your other jotform I found both are very similar except for the create a pdf download as you have mentioned. I also tried changing the font size and removing the css to check it further but the result is the same in my test. In addition to that I did a font size test on a brand new form set to 12 & 14 but those were fine.
Currently, it seems to me that there might possibly be a pdf output bug on your jotform and that may explain why it's happening on this particular jotform and not the other. I've sent a bug report to have this investigated furthermore.
