Import form just flashes "Generate form" real quick, disappears, and then does nothing

Guys, problem is back today.  It's now different though.  It doesn't say "can't find page/form" when try import, it just flashes "Generate form" real quick, disappears, and then does nothing.  Please check and let me know if you need more details.
