A field is not showing up when I embed the form on my webpage

    I discovered another concern I cannot overcome on my own:  in JotForm, the required field, labeled Name*, is not appearing on my online published form.  Am I embedding the JotForm incorrectly?  My University ITS team does not have much familiarity with jotform.  I've copied the embedding trail and pasted that in the same place I've pasted three times before (this is my fourth form)...
     
    Please help me help our students sign-up for our awesome program!

    You have configured a condition that shows this field only when "Community Member (Non-NIU affiliated)" is selected in the Select Status field.

    This means that if the above condition is not met the field will be hidden, if you want to always show the Name field then you need to remove it from the mentioned condition.
