I have built a new form and when I try out a response to it the notification email I get doesn't have anything showing in First Name and Last Name fields even though I filled them out and they are required fields

When I click on the PDF link in the email with the response I can see the names.

I can't see all the text.   There are liability statements at the end of my form and they are not coming up on the pdf response.   We really needs those to appear.   Yes I have selected to show header and text. from the "Get PDF" options.

Thanks in advance for your help

Since we can only assist with one question per thread I have moved your issue regarding PDF to a new thread Mary. You can find it here: and we will be replying to it shortly.

Now in regards to the emails.

I see that the email was sent to 2 addresses at the same time and that the email that was sent to Caroline's email address was received properly, but not the one that was sent to you.

You can see both of them in the email logs.

What I would recommend is to delete that email notification that is sending data to your email and then recreating a new one and testing it out. It should be working properly for you after doing that.

Do let us know Mary if it however does not by some chance.

I'm still having the same problem on all the forms I am creating.   Not all of the fields that have been filled in are showing up as filled in on the email notification.   I did as Ben suggested and stripped out the email addresses on my form at

Now I am getting two emails coming in to the same address.  One of them shows the pdf link I added but the "First Name" field is empty in the email body

The other email coming in at the same time has no pdf link BUT the "first Name" field is filled in.

Why is it now coming in to the one entered address twice?

And why do the two emails coming in differ?

    Hello Mary,

Let me first address your issue on why some form fields does not gets displayed in the submission emails.

Please be noted that when you edit your email alert, it becomes dirty. That means after the email template is edited, any changes you make in your form like (add/edit & delete form fields) will not reflect in the notification email template. You will have to manually make the changes in your email template as well.

The best way to solve this problem is to delete the existing email alert and add a new one. Please please be noted that a new email alert will always have all the form fields in the template.

I did check the email alerts you have added in your form and it seems I was able to figure out the issue. It seems you have edited the field name of "First Name" field which actually has a space in field name. Please check the screenshot below:

I am not sure how you were able to save it but space is not allowed in field name and that is the reason why you do not see the first name value in email alerts.

It is possible to edit the name and then update all the email alerts to show correct field names. I would suggest you to delete this field and add a new one and that should solve your problem. You will also have to delete the email alerts and add new one as well.

The following guide should help you :

    Delete email alert:

    Add email alert:

    Hope this helps.

Do get back to us if you have any questions.

    Thank you!
