Can I create a word document and transfer into jot form?

I am not sure if I have understood your question correctly. Do you want to embed a word document in your form? 

Unfortunately, it is not possible to embed and display your word document in form but, one possibility is to convert your word document to PDF and then take advantage of our "PDF Embedder" widget to display in with in your form.

Alternatively, you can upload your word document to your own server and then add a hyperlink in your form by adding a "Text" field.
