    How can I import my Ebook Form from Acrobat InDesign

    I have a pdf epub magazine that has a form in the back to collect survey data. How do I do this? Can I use Jotform to collect the data? I build the form using Acrobat XI or InDesign.

    I recommend you also create your JotForm account first. You can use this link https://www.jotform.com/signup to signup using your email account.

    I am not sure how your epub form look at the moment, but from what I searched about it, you will mostly likely need to re-create it using the From Builder.

    If you can share to us your epub forms, we will be more than glad to assist you create it in JotForm.

    So you cannot use Jotform for form processing on an existing form in a pdf.  You cannot capture data from forms in PDF's.

    If you have a website, you can try to export your form to HTML and publish it.
    Then you could try to import the published form using its URL as described here: http://www.jotform.com/help/42-How-to-Clone-an-Existing-Form-from-a-URL 

    Also, you can check our templates section and our theme store, maybe you can find a ready-to-use form that would look similar to the one you have in your magazine.
